Creating an account for your child or dependent?

Mobile

  • At the bottom of the screen click on the heart named “My Health”.

  • Then click on your name at the top of the screen.

  • Select “Add a family member”.

  • Follow the instructions and you will be all set.

  • Once you have completed the instructions go back to the schedule page to schedule your appointment.

Desktop

  • In the upper right hand corner click on your initials.

  • Click on “Switch Patient”

  • Select “Add a family member”.

  • Follow the instructions and you will be all set.

  • Once you have completed the instructions go back to the schedule page to schedule your appointment.
